Which Prudential line you need

Prudential publishes two fax numbers, and both are annuity service lines. (800) 576-1217 appears in Prudential’s FAQ for annuity service requests, including required minimum distribution elections. (800) 207-7806 appears on the contact page, for contracts that were reinsured to Fortitude Re and are serviced by Prudential as third-party administrator.

Which one applies is decided by your contract, not by the product name. A contract owner has to confirm which service line holds their contract before sending, because the two queues are separate.

Two redirections matter here. Prudential’s retirement plan recordkeeping business moved to Empower, so 401(k) and workplace plan paperwork is not faxed to Prudential; the Empower guide covers that routing. What people fax to Prudential

The annuity service lines take contract-level paperwork:

• Required minimum distribution elections and waiver requests
• Automated withdrawal setup and changes
• Other annuity service forms that name a fax destination in their instructions
• Fortitude Re contract service paperwork, for reinsured contracts

One caveat to read on your own form: Prudential’s FAQ notes that the Automated Withdrawal Form carries its own fax contact printed on the form. Where a form names a number, that number wins over the general one.

An annuity service request with identification usually runs three to six pages, which is at or just past the free monthly allowance.

These lines take documents in, they do not answer

Both numbers are service intakes. Pages arrive, a service representative works the request, and Prudential responds by mail, by phone, or in your account. Nothing comes back by fax and there is no fax acknowledgment.

Verify the number before you send

• Confirm which service line holds your contract before sending. Prudential-issued and Fortitude Re contracts are serviced through different queues.
• Read your own form. Where a Prudential form prints a fax number, that is the destination for that form.
• If the paperwork is for a 401(k) or another workplace plan, check whether Empower now services it.
• Never fax wire instructions, a full bank routing and account pair, or login credentials. A sent fax cannot be recalled.
