Which U.S. Bank fax line you need

U.S. Bank does not list a general fax number on a contact page. Each of these comes from the knowledge-base article for one task.

Card paperwork. Correspondence and dispute documentation for a U.S. Bank credit card go to (866) 568-7729.

Proof of insurance on a financed vehicle. The declarations page from your insurer goes to (866) 804-8764. This is the line to use when the bank has written asking you to confirm coverage.

Escrow tax documents. Property tax bills and homestead exemption paperwork go to the Home Mortgage tax department at (877) 903-6972.

A W-9. After a backup-withholding notice, the completed form goes to (314) 418-3530.

A verification of deposit. Medical and public assistance agencies send their requests to (866) 338-4251. This is an agency line: if you have been asked for a verification of deposit, hand the request to whoever asked for it.

Some things route elsewhere. U.S. Bank publishes no fax for estate or power of attorney documents; its wealth-management POA affidavit names none, and those go through a branch or by phone. Mortgage payoff quotes are the reverse of what people expect: U.S. Bank will fax a quote out to you on request, but publishes no number to fax a request in.

What customers send to these lines

Each of these lines expects one document, and usually a short one:

• An insurance declarations page naming U.S. Bank as lienholder on a vehicle
• A property tax bill or a homestead exemption approval for an escrowed mortgage
• A completed Form W-9 after a withholding notice
• Dispute documentation such as receipts, a merchant response, or a written explanation for a card charge
• An agency verification-of-deposit request with the customer authorization attached

Each number is an intake queue. U.S. Bank processes the document and follows up in online banking, by phone, or by mail. Nothing comes back by fax and no receipt is issued on arrival.

• Do not send estate or power of attorney documents to any of these lines. U.S. Bank publishes no fax for either, and there is no queue expecting them.
• Never fax wire instructions or your online banking credentials. U.S. Bank does not ask for them by fax, and a payment-redirection request that arrived by email is a fraud pattern, not an errand.
• If a representative gives you a number over the phone, use theirs and note who gave it to you.
